Dr. Jordan Kemere is an internal medicine physician and assistant professor at Baylor College of Medicine. In this session, Dr. Kemere reviewed practical tips for transitioning from a pediatric to an adult healthcare framework. She discussed strategies for genetics providers and also patients/caregivers to prepare several years in advance of transfer. Lastly, she talked about self-management skills, increasing knowledge, and tips and tricks at the time of transfer as you work with adult providers.
